Cancellation and Refund Policy

If a member is dissatisfied with the plan and wishes to cancel, the member must send a written cancellation letter, together with their membership card(s) to:

Upon cancellation Avia Dental plan will stop collecting membership fees in a reasonable amount of time, but no later than 30 days after receiving a valid cancellation notice. In the event of cancellation by either party, Avia will make a pro-rata reimbursement of the periodic charges to member for the unexpired portion of the term. The enrollment fee is non-refundable except where prohibited by law.

For residents of CO, FL, IN, LA, MD, MO, NH, NY, OH, TN, TX, UT, VT, and WV, if you cancel within the first 30 days after activation you will receive a full refund, except for the enrollment fee where permitted by law. This refund will include the enrollment fee for residents of TN and VT.

Renewal Conditions

By joining a plan, you are authorizing Avia Dental Plan to bill your credit card or checking account for the plan you have selected. This charge shall renew until you notify Avia Dental Plan in writing of its cancellation. By joining, you indicate you have read the terms and conditions of the plan. This plan will automatically renew at the end of your membership term, and your credit card or bank account will be automatically charged or drafted for the appropriate amount.

Termination Conditions

Avia Dental Plan reserves the right to terminate plan members from its plan for any reason, including non-payment. If Avia terminates the plan or your membership for a reason other than non-payment, you will receive a pro-rata refund of your membership fees.

This site is owned and operated by Avia Dental Plan ® The trademarks, design, graphics, text, logos, and service marks (“Marks”) displayed on this site are the property of Avia Dental Plan ® with exception to those of Avia Dental partners and clients. Users are prohibited from using any Marks for any purpose including, but not limited to use as metatags on other pages or sites on the web without the written permission of Avia Dental or such third party which may own the Marks. All information and content including any software programs available on or through the Site (“Content”) is protected by copyright. Users are prohibited from modifying, copying, distributing, transmitting, displaying, publishing, selling, licensing, creating derivative works or using any Content available on or through the Site for commercial or public purposes. Avia Dental, under no circumstances, shall be liable for any damages that result from the use of the materials in this site.

 Terms and Conditions

Avia Dental Plan ® is not insurance. It is a discount membership plan offered to individuals who live in various states within the dental network. Members of Avia Dental Plan ® receive discounts on dental, vision and prescription services. There is a minimum one year contract.

The fee schedule pertains to participating general dentists only. The fee schedules are current prices by participating providers and are subject to change without notice. Any procedures performed by a non-participating provider are not discounted. Any procedure delivered which is not listed on the fee schedule is discounted 20-25% depending on area. In cases in which you are referred to a participating dental specialist, you will generally receive 20-25% off their usual and customary fees. Please verify benefits with each individual provider. Specialists may not be available in all areas. Some fees do not include lab costs which are the member’s responsibility. Consult with your provider to obtain the fee for any procedure not listed. Some providers may charge for missed or broken appointments if no prior notice is given.

Providers are subject to change without notice and benefits may vary in some states. It is the member’s responsibility to verify that there is a provider in their area who participates in the plan. At any time, Avia Dental Plan ® has the right to eliminate a participating provider from the network. Avia Dental Plan ® cannot guarantee the continued participation of any provider. If he or she leaves the plan, you will need to select another provider. Work in progress, after enrollment on the plan, must be completed by the provider who started the work.

Providers contracted by Avia Dental Plan ® are solely responsible for the professional advice and treatment rendered to members and Avia Dental Plan ® disclaims all warranties, express and implied, with respect to such services. Members agree that Avia Dental Plan ® and its administrators shall have no liability for providing or guaranteeing service or the quality of serice rendered at participating providers.

By enrolling online, you are authorizing Avia Dental Plan ® to bill your credit card or checking account. This charge shall remain in force until you notify Avia Dental Plan ® in writing. By enrolling online, you are agreeing to the terms and conditions of the plan and adopting it for a minimum of one year. Unless otherwise advised the automatic renewal of your dental plan will automatically renew at the end of your membership term, and your credit card or bank account will be automatically charged or drafted for the appropriate amount. Avia Dental Plan ® reserves the right at any time after receipt of your order to accept or decline for any reason.

By electronically accepting these terms and conditions, the member agrees and understands that they are entering into a binding legal contract.
